A medical record is also known as a medical chart or health record. A medical record is an organized documentation of an individual’s medical history and details. The term ‘Medical record’ is used both for the physical folder for each individual patient and for the body of information which comprises each patient’s health history.
Medical records are guarded as it’s a set of individual documents and may contain delicate health issues. It is more of a medical tracker which is governed by legal policies related to access and storage of medical records. The governing laws in this regard are different for different countries. Medical records serve a global purpose with the advancement in medical research which many a times combines study done by scientists and health workers from multiple countries.
There are various advantages of these medical charts and some aspects are discussed below
Healthcare Planning and Care: – Medical records not only form a database, but also serve as a base for patient care and planning. It forms a bridge between health care provider and a health professional to provide good health care to patients.
Medical Epidemics and Outbreaks: – Medical outbreaks are not common but they do occur. Medical help to control the spread of a disease and steps to quarantine it.
Study and Research: – Health ministry and even many government bodies support the study and research related to antibiotics and medicines to provide affordability. Local physicians related to the concerned hospitals and resident doctors get help through the documented medical reports. Mostly identities are held while accessing other information related to research.
Hospital Auditing: – It assists in protecting the legal interest of the patient and the health care providers responsible for the patient’s care, and documenting the care and services provided to the patient.
Apart from these uses, medical serve many other purposes as well.
Medical contain the medical history and apart from that contain date of birth, blood group, sex, emergency contacts, results, dates and details of screenings and tests, major illnesses and its details, allergies and medical history which might run through the family.
In the event of a medical emergency involving a non-communicative patient, consent to access medical records is assumed unless written documentation has been previously drafted like in cases of advance directives. Konuten the disposals of records have legal issues and laws attached with it. Also, medical are subject to legal revisions from time to time.
It can be concluded that the field of records is growing and this eventually helps the individual involved and the society in general.
